Bishop McHugh PreK Summer Camp 2018 Waiver

PLEASE NOTE that, by signing this release, you are not only requesting that your child participate in the summer camp and consenting to such participation, but you are also waiving any and all claims that might arise out of such participation and agree to provide indemnity for such claims.

We, as parents/guardians hereby consent and give permission for our child,

to participate in the activities described in The Bishop McHugh PreK Summer Camp 2018. In consideration of the permission granted by The Bishop James T. McHugh Regional School, Inc. for our child to participate in this activity, we release any and all claims against The Bishop James T. McHugh Regional School, Inc., The Diocese of Camden, New Jersey, and their respective agents, servants, employees, officers, trustees, administrators and volunteers, for damages and/or injuries to us or our child which may arise from participation in this activity and agree to indemnify and hold these entities harmless from and against any claim or claims brought by or on behalf of our child or by or on behalf of any other person arising out of or in any way connected with our child’s participation in the activity.

Payment is handled in advance at a rate of $175 per week or $35 per day. Please indicate your selections above so that we can plan for staffing.  Registration is needed by May 15, 2018 to confirm your spot.  If we do not have sufficient enrollment by May 15, refunds will be available.  Please make checks payable to Bishop McHugh School (with names of child(ren) indicated and mail to Bishop McHugh School, 2221 Route 9 North, Cape May Court House, NJ 08210, attn.: PreK Summer Camp.
